3. The patient or his/her family members are responsible for recruiting donors.
Persons asked to participate should be well known by the patient or family, should be asked but
not forced to donate, and must satisfy at least the following criteria:

Weigh 110 pounds or more, and be at least 17 years of age
|

Never have had cancer, hepatitis or be a member of a high-risk group
for exposure to the HIV virus (virus that causes AIDS)

| |
Be free of all illness and infection
|
|
| |
Not have had any major surgery, received blood products, had a tattoo,
ear or skin piercing, acupuncture, accidental needle stick, or come in contact with someone
elses blood within the last 12 months

Successfully undergo a limited physical examination and
detailed medical history interview prior to donation

4. Charges for the special handling of directed donor units are billed
to the intended recipient. This fee is charged for each unit of directed donor blood
requested. These fees may not be covered by insurance plan(s). In addition, the standard
fees when units are cross-matched and transfused will be charged to the patient.
5. The husband and/or paternal relatives will not be accepted for
female patients of childbearing age.
6. Appointments should be made in advance by calling your nearest
blood donor center.
